Registration (Регистрација)

All registrations (participants, fellows/students and accompanying persons) for the Congress must be submitted using the official Registration Form.

Microsoft word template. MS word document is available on our site www.endocrinology.org.mk, in the field of registration. Please download the form, fill and send it back to: info@endocrinology.org.mk.  

Please use one Registration Form for each person registering. Only fully completed Registration Forms will be accepted.

Registration Fees
Regular Participants            80€
Fellows/Students                  40€
Accompanying person        40€

The registration fee for regular participants and fellows/students includes entry to all official Congress sessions, the exhibition and poster area and the opening ceremony. It also includes the Congress bag, containing on-site Congress publications.
To qualify for the student registration fee, students and residents must be able to present proof of full-time enrolment at an accredited university or college at the time of registration. Please attach a photocopy of a current registration card or a letter from your department administration office.
Accompanying persons are entitled to visit the exhibition and to attend the Welcome Reception. However, they are not allowed to attend any scientific session and are not eligible for a congress bag.

Letter of Invitation
Individuals requiring an official letter of invitation in order to obtain a visa and authorization to attend the Congress, should contact the Organising Committee. The letter of invitation does not financially obligate the Congress organizers in any way. All expenses incurred in relation to the Congress are the sole responsibility of the participant.

Letter of Confirmation
Registration to the Congress will be confirmed by sending out a letter of confirmation as soon as the fully completed Registration Form as well as the payment have been received by Organising Committee. Participants must present this confirmation letter at the registration counter as proof of their registration.

Cancellation of Registration
Notification of cancellation must be made in writing and sent to Organising Committee with a reference to a bank account. A cancellation fee will be charged depending on the date of cancellation. All refunds will only be made after the Congress.

Methods of Payment
The registration fee must accompany all Congress registrations. Payment should be made in advance and in EURO (€) only, using bank transfer to:

Сите учесници (доктрои, специјализанти, студенти и придружни особи) потребно е да ја пополнат официјалната регистрациона листа на Конгресот.

Котизацијата е потребна за влез на официјалните конгресни сесии, комерцијаланта изложба, како и и на свеченото отворање и затворање на конгресот. Исто така вклучува и конгресни материјали.

Специјализантите и студентите потребно е да приложат официјална потврда за нивниот статус заверена од соодветната институција (специјализантска книшка, индекс или поврда).
Придружните особи можат да ја посетат комерцијаланта изложба, како и да присуствуваат на свеченото отворање и затворање на конгресот. Тие не можат да присуствуваат на конгресните сесии, како и да добијат конгресни материјали.

Официјална покана
На лицата на кои им е потребна официјлна покана за присусво на Конгресот, потребо е да го контактираат организациониот комитет. Официјалната покана не ги покрива финансиските трошоци на конгресот.

Писмо за потврда
Регистрацијата ќе биде завршена кога ќе биде испратена комплетна регистрациона листа и откако ќе биде извршено плаќањето на котизацијата. Доколку котизацијата ја покрива фармацевтска компанија, регистрирана во Македонија, Ве молиме да приложите соодветно писмо. Потоа ќе биде испратено писмо за потврда за регистрацијата. Ова писмо е потребно да се прикаже на рецепцијата на хотел Метропол во Охрид.

 
Откажување на регистрација
Откажувањето од регистрација е возможно да се направи со напушан документ пратен до Организациониот Комитет. Срествата ќе бидат делумно вратени по завршување на Конгресот.
